Dosage Calculation Formulas

Every equation the calculators on this site run, with the paper it came from where it has one. The citations are read out of the same code the calculators use, so they cannot go stale independently of the maths.

Dose and volume

Dose to volume

volume (mL) = dose (mg) ÷ strength (mg/mL)

The relationship the whole site runs on. Neither quantity converts to the other without the strength.

Strength from an amount in a volume

strength (mg/mL) = amount (mg) ÷ volume (mL)

How most oral liquids are labelled: 250 mg in 5 mL is 50 mg/mL.

Percentage strength

strength (mg/mL) = percentage × 10

A percentage is grams of drug in 100 mL, so 1% is 10 mg/mL.

Ratio strength

strength (mg/mL) = 1000 ÷ the second number

1:1000 is one gram in 1000 mL, which is 1 mg/mL.

Units to volume

volume (mL) = dose (units) ÷ strength (units/mL)

International units measure activity, not mass, so they never convert to milligrams.

Mass unit ladder

1 g = 1000 mg = 1,000,000 mcg

Fixed for every drug and every product, unlike the dose-to-volume relationship.

Working out the dose

Weight-based dose

dose (mg) = mg/kg × weight (kg)

Whether the mg/kg figure is per dose or per day changes the answer by the number of doses.

Divided daily dose

dose per administration = daily total ÷ doses per day

Applies only when the order is written per day.

Tablets per dose

tablets = dose ÷ strength of one tablet

A result that is not a whole or half tablet means the stock strength cannot make the dose.

Infusions

Pump rate

mL/hr = volume (mL) ÷ time (hr)

What a pump is programmed with. The giving set is irrelevant to it.

Infusion time

time (hr) = volume (mL) ÷ rate (mL/hr)

The same relationship read the other way.

Gravity drip rate

gtt/min = volume (mL) × drop factor (gtt/mL) ÷ time (min)

The drop factor is a property of the giving set and is printed on its packet.

Dose rate to pump rate

mL/hr = (dose rate × weight) ÷ bag strength (mcg/mL) × 60

A dose rate and a pump rate are different quantities. Only the second goes into a pump.
